To order boots under the HAIX® OSHA Outreach Trainer Boot Program, please download this form and be sure to include the following information:
- Boot model and size desired*
- Phone number where you can be reached
- Shipping and Billing Address (if different)
- A copy of the front of your Outreach trainer card**
- A copy of a Photo ID**

**For verification purposes, you must provide us with a copy of the front of your Outreach trainer card and a photo id.
